Some Frequently Asked Questions…


“Form Creation” is the online interface that enables you to create new forms and modify existing ones that power your SmartCAT Forms.
If you’re having trouble logging in it could be one of the following issues:
Forgotten Password: Please visit and follow the steps to reset your password.
Expired License: Trial usage is good for 30 days upon sign-up, your trial license might have expired. Please contact your SmartCAT Sales Representative or click here to purchase your SmartCAT Forms license.
Maximum Device Limit: A single user license can only be used on a maximum of 3 different devices. Please ensure you are logged out of one before attempting to log into another.
In order to switch to a different user, your device will need to have internet connection. This is so the new user can log in and download the forms and records assigned to the particular user.
All media files are stored either in Cloud Files (hosted by Rackspace) or a user defined media location. The URL of each media file is stored in a data file that requires a SmartCAT Forms Username and Password with viewing rights to that form to retrieve. The name of the file is generated randomly.
SmartCAT Forms databases live in a secure data center that are tested yearly through a SSAE16Type II (formerly SAS70 Type II) yearly audit. For more information please contact our support team, below.
SmartCAT Forms’ dedicated database provides a heightened level of data security, flexibility and customization within the Cloud. It includes access to a robust API which supports both inbound and outbound requests.
